Problem resolution Information

Federal law gives every hospital patient the right to be informed of how to submit a complaint to the hospital relating to his/her care or relating to the belief that he/she is being discharged from the hospital prematurely. Each patient has the right to be informed of how the complaint will be considered including the response and resolution process developed by the hospital. The complaint resolution process is part of the hospital’s confidential Quality Improvement Program.

If you have a concern or question First Texas Hospital will work with you to resolve it promptly. You may address you concern in the following ways:

Promptly speak with your nurse or another health care professional involved in your care. However, if you feel an issue is not being addressed appropriately, or if you need additional assistance, please contact our Director of Quality Management at (346) 206-2330.

If you feel that your issue is not being resolved or being addressed by the hospital, you may also phone or write the Texas Department of State Health Services, Health Facility Compliance Division, 1100 West 49th Street, Austin Texas, 78756. You may also call 1-888-973-0022 to ask questions or report a complaint.

Patients who want to address any concerns with The Joint Commission, may call the hotline at 1 (800) 994-6610, 24 hours a day, seven days a week. Staff members are available to answer calls at this number, weekdays between 8:30 AM and 5:00 PM Central Standard Time.

Grievance Information

The satisfaction of our patients and visitors is important to us. If you have a concern or question you may speak with your nurse or department manager at any time.

Should you have further concerns about patient rights, safety, or any other complaints or grievances, please contact our Director of Quality Management at (346) 206-2330. You may submit a grievance without retaliation.

If you are not satisfied with the resolution, you have the right to contact the Texas Department of State Health Services, Customer Service Representative, 1100 West 49th Street, Austin, TX 78756 or at 1-888-963-7111.

In addition, patients who wish to address concerns with Joint Commission may contact Customer Service at 1-(630)-792-5800 or at [email protected]. Complaints may be directed to 1-800-994-6610 or at [email protected].
